This Website is owned and operated by The University of Hong Kong Clinical Trials Centre ("HKU-CTC"). HKU-CTC has been authorized as the central platform for management and coordination of clinical studies under HKU and its affiliated teaching hospital –Queen Mary Hospital (QMH). HKU-CTC is devoted to partnering with clinical investigators, research organizations and the healthcare industry in advancing human healthcare by promoting, attracting and facilitating clinical studies. | A. | Submission of this form to HKU-CTC indicates your organization's intention to request HKU-CTC to perform research administration services (ie. initial ethics submission, budget calculation and contract preparation) necessary for preparation for the study, and your organization's agreement to pay HKU-CTC the relevant research administration fees which are subject to adjustments from time to time. You may contact us for inquiries about the prevailing fees. |
| B. | Information provided on this form by your organization may be used for making submissions to the relevant IRBs. By submitting this form to HKU-CTC, your organization warrants that the information herein is correct and accurate to the best of your company's knowledge. |
